I understand that the services I will receive from Roberta are not intended as a substitute for professional mental health care or medical care or to diagnose, treat or cure mental health or medical conditions. I also understand that Roberta is not acting as a mental health counsellor or medical professional.

I understand that the services offered by The Mindful Mum of London are not a substitute for any form of counselling, psychotherapy, mental health care or substance abuse treatment, and I will not use it in place of therapy.

For legal purposes, I understand that coaching is currently an unregulated industry and that Roberta is not “licensed “ by any UK body even though the sessions may take place in the UK.

I understand that my wellbeing, choices and decisions are my full responsibility during the time that I receive any kind of support from Roberta.

I understand and agree that I am fully responsible for my well-being during my coaching sessions, and subsequently, including my choices and decisions.

I understand that any advice offered by Roberta are solely for the purpose of helping me to achieve any goals I create during my sessions. I have the ability to give informed consent, and therefore give such consent to allow Roberta to assist me in achieving such goals and understand that results are not guaranteed.

I understand that Roberta will protect my information as confidential unless I specifically state otherwise and give this in writing. I also understand that this confidentially agreement may be limited if I threaten to harm myself, someone else, or report any form of abuse or neglect. In this case, further action will be taken if necessary. I understand that if Roberta is ordered by a court to provide any information or testify she will do so to the extent the law requires.

I understand that the use of technology and social media is not always secure and I accept the risks of confidentiality in the use of these technologies and platforms.

I hereby release, waive, acquit and forever discharge Roberta, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by Roberta as a result of the advice given by my Roberta or otherwise resulting from the coaching relationship contemplated by this agreement.

I declare that no promise, inducement or agreement that is not expressed within this agreement has been made to me in order for me to sign this agreement. This agreement binds my heirs, executors, personal representatives, successors, assigns and agents.
